Application for Incomplete Grade

A grade of incomplete may be issued to a student in a course for which the student has been unable to complete the requirements due to extenuating circumstances. An incomplete grade is assigned at the course instructor's discretion and only when exceptional circumstances beyond the control of the student have prevented the student from completing the final assigned work or examination.

All requirements to satisfy the incomplete grade must be completed no later than ten weeks following the last day of the term in which the incomplete is issued. This form must be submitted, with all required signatures, prior to the last of the term (last day of final exams).
