Instructor approval is required to add courses after the beginning of a term. Refer to the Academic Calendar for published registration deadlines. A separate form must be submitted for each course.

Read each field carefully and be sure to enter the information that is requested. Entering incorrect email addresses for a FACULTY ADVISOR or an INSTRUCTOR will prevent your form from being delivered to them for approval.
