Academic Review Appeal Form

Undergraduate students are expected to make continuous progress toward their educational goals. Student academic performance is reviewed following each regular semester (fall and spring). Academic standings are not updated following summer and non-standard instructional periods. Continuing academic problems may result in academic probation and suspension with restrictions on enrollment and extracurricular activities. This completed form and all supporting documentation must be submitted by the deadline on June 9, 2023, or the appeal will not be considered by the Committee.


Your form will be forwarded to the Center for Advising & Academic Success, Business Office, Financial Aid, Dean of Students, and Registrar's Office.
